What Causes a Sudden Spinning Sensation?

If you suddenly feel like the room is spinning — even though you’re standing still — you’re experiencing true vertigo.

Vertigo is not simply “feeling dizzy.”

What Are the Most Common Causes of Sudden Vertigo?

1. Benign Paroxysmal Positional Vertigo (BPPV)

BPPV occurs when calcium crystals (otoconia) become displaced within the inner ear.
It typically causes brief spinning when:

  • Rolling in bed
  • Looking up
  • Bending forward

This is thought to be the most common cause of sudden vertigo. Although we see this commonly over-diagnosed in people that actually have central vertigo.


2. Vestibular Neuritis

A viral inflammation of the vestibular nerve that can cause:

  • Severe spinning
  • Nausea
  • Imbalance lasting days

Sometimes vertigo is not coming from the ear at all.

It can be due to dysfunction in:

  • Brainstem vestibular nuclei
  • Cerebellar timing circuits
  • Ocular motor integration pathways

4. Dysautonomia or POTS

If spinning occurs with standing and improves with sitting or lying down, the cause may be autonomic instability.

  • Severe headache
