You cannot sleep, but you cannot fully wake up either. Your digestion is unpredictable. Your heart races for no reason. You feel tense all the time, even when nothing is wrong.

This is more common than most people realize, and it has a name: chronic sympathetic dominance. Understanding what is happening in your nervous system is the first step toward actually fixing it.

What Is Fight or Flight?

Fight or flight is your body’s built-in survival response, controlled by the sympathetic branch of your autonomic nervous system. When your brain perceives a threat, it triggers an immediate cascade of physiological changes designed to help you survive.

Adrenaline and cortisol flood your system. Your heart rate climbs. Your breathing becomes shallow and fast. Blood is redirected away from your digestive organs and toward your muscles. Your immune system downregulates. Your brain sharpens its focus on the immediate threat and shuts out everything else.

In a true emergency, this response is lifesaving. It is the reason humans survived for thousands of years in genuinely dangerous environments.

The problem is that your nervous system cannot tell the difference between a physical threat and a chronic one. It responds the same way to a car accident as it does to years of unrelenting stress, chronic pain, illness, or trauma. And when the activation never fully resolves, the body stays in emergency mode indefinitely.

This is called chronic sympathetic dominance, and it is far more than a mental health issue. It is a physiological state that affects every system in your body.

What Is the Autonomic Nervous System?

To understand why fight or flight affects so many different symptoms, it helps to understand what the autonomic nervous system actually controls.

The autonomic nervous system (ANS) is the branch of your nervous system responsible for all the functions your body runs automatically: heart rate, blood pressure, digestion, breathing, immune response, hormone regulation, temperature control, and more. You do not consciously control any of these things. Your ANS handles them without your input.

The ANS has two primary divisions that work in opposition to each other:

The sympathetic nervous system activates fight or flight. It accelerates, mobilizes, and prepares the body for action.

The parasympathetic nervous system activates rest and repair. It slows the heart rate, restores digestion, promotes sleep, and allows the body to heal.

In a healthy nervous system, these two branches balance each other. You shift into sympathetic activation when you need it and back into parasympathetic recovery when the threat has passed. The problem develops when the sympathetic system becomes chronically dominant and the parasympathetic system cannot fully engage. When that balance breaks down, nothing in your body works the way it should.

Signs Your Nervous System Is Stuck in Fight or Flight

Because the autonomic nervous system touches nearly every function in the body, chronic sympathetic dominance produces a wide and often confusing range of symptoms. Patients frequently see multiple specialists for what are actually different expressions of the same underlying problem.

You Cannot Wind Down at Night

Sleep requires a shift into parasympathetic dominance. Your heart rate needs to slow, your breathing needs to deepen, and your brain needs to downregulate its alertness. When your sympathetic system is chronically activated, that shift struggles to happen. You lie down exhausted but feel wired. You wake in the middle of the night with your mind racing. You never feel fully rested.

You Startle Easily or Feel Constantly on Edge

A nervous system in fight or flight is hypervigilant by design. It is scanning for threats constantly. This shows up as an exaggerated startle response, a persistent sense of unease, irritability, or a feeling of dread that has no clear source. Many patients describe feeling like something bad is about to happen, even when their life circumstances do not warrant it.

Your Heart Races Randomly

The sympathetic nervous system accelerates the heart as part of the fight or flight response. When it stays activated, episodes of racing heart, pounding pulse, or palpitations can occur even at rest or during mild activity. This is one of the symptoms most often misattributed to anxiety or cardiac issues when autonomic dysregulation is the actual driver.

Your Digestion Is Unpredictable

Digestion is a parasympathetic function. When your body thinks it is in danger, digestion is considered nonessential and gets downregulated. Chronic sympathetic dominance is closely linked to irritable bowel syndrome, bloating, constipation, diarrhea, nausea, and general gut dysfunction. This is also why the gut-brain connection is so significant: a dysregulated nervous system directly disrupts digestive health.

Your Hands and Feet Are Always Cold

The fight or flight response redirects blood flow away from the extremities and toward the large muscles needed for survival. Chronically cold hands and feet, even in warm environments, can be a sign that your body is perpetually in this redirected state. This symptom is also common in dysautonomia and POTS, both of which involve autonomic nervous system dysfunction.

You Are Exhausted but Cannot Rest

This is one of the most common and confusing symptoms of chronic sympathetic dominance. The body is burning enormous energy maintaining a state of high alert, which produces deep fatigue. But because the nervous system is still activated, true rest is elusive. Patients describe feeling simultaneously drained and unable to relax, often for years at a time.

You Have Brain Fog or Difficulty Concentrating

While acute stress sharpens focus on immediate threats, chronic sympathetic activation has the opposite effect on higher cognitive function. Sustained cortisol elevation impairs memory consolidation, reduces processing speed, and contributes to the mental cloudiness that many patients describe as brain fog. This symptom is frequently misattributed to thyroid issues, depression, or age.

You Have Chronic Muscle Tension, Jaw Clenching, or Tension Headaches

The body braces for impact in fight or flight. Muscles tighten, particularly in the neck, shoulders, and jaw. When this state is chronic, so is the tension. Many patients with sympathetic dominance grind their teeth at night, carry persistent tightness in the upper back and neck, and experience frequent tension-type headaches as a result.

Your Breathing Is Shallow

Deep, diaphragmatic breathing is a parasympathetic function. Shallow, chest-based breathing is sympathetic. Most people in a chronically activated state have adapted to shallow breathing without realizing it, which further reinforces the sympathetic state. Breathing and nervous system regulation are bidirectional: your breath both reflects and influences which branch of the ANS is dominant.

Why This Does Not Get Better on Its Own

Chronic sympathetic dominance tends to be self-reinforcing. The longer the nervous system stays in fight or flight, the more sensitized it becomes to perceived threats, and the harder it becomes to shift back into parasympathetic recovery. Sleep deprivation worsens stress reactivity. Digestive dysfunction impairs nutrient absorption. Fatigue reduces the body’s capacity to regulate. The cycle compounds.
